When applying Levels to an image...

When applying levels to an image object, that has been pasted into a vector object, that already has a black & white filter applied... Let just say the results are less than desirable.

What was the image format and color mode? Also: what OS you have? Image filtering uses compute, and that might also cause issues on some systems. You could try disabling compute for the document in View -> Display -> Display Options -> Images and uncheck Compute Filtering.
Also: there have been reports of issues with grayscale image display.
