I have noticed there are times when I double-click on a path using the node editing tool expecting it to create a new node at that location, but it does not. Sometimes it takes several attempts to get it to create one. Other times I have to zoom in closer to the path to get that the new node to appear on the path. Here is a video of this happening. It worked correctly for a few and then you'll see it stopped before a third set of tries worked again.
