option modifier with flip tool fixed but has occasional issue

Thank you for fixing the option to copy an selected object by using the option modifier while clicking on the symbol at the top left of the UI. I noticed there are still occasional times when the copy ends up in an odd location, not right next to or overtop the original like expected. Sometimes just restarting the file fixes the issue.
If I run into this issue again, what would you need from me to potentially diagnose the cause?
